Bowel Diagnostics & Therapy Patient Referral Program

Terms and Conditions

Eligibility: 

·  This promotion is open only to current patients of Bowel Diagnostics & Therapy (BDT) who have completed their initial AND functional bowel assessment appointment. 

·  Referred friends and family members must be new patients who book and have attended their initial AND functional bowel assessment appointment to qualify for the reward. 

·  The promotion is available to patients aged 18 and over. 

Promotion Period: 

The referral program starts on Monday 4th November 2024, has no set end date and will continue until advised otherwise. BDT reserves the right to modify or terminate the promotion at any time without prior notice. 

Referral Process: 

·  To participate, existing patients must refer friends or family members to book an initial appointment with BDT.

·  The referred individual must provide the referring patient’s name upon completing the new patient form. 

·  The referred individuals must complete the initial AND bowel assessment for the referral to be valid. 

Rewards: 

·  For each valid referral, the referring patient will receive a $50 Coles Gift Card once the referred patient has attended their initial AND bowel assessment appointment.

·  There is no limit to the number of referrals a patient can make.

·  Gift cards will be processed and sent out within four weeks of the referred patient’s appointment, subject to verification of the referral details. 

·  The $50 Coles Gift Card cannot be exchanged for cash or any other product. 

Referral Verification and Reward Distribution: 

·  Dedicated staff will review referral sources weekly to verify eligibility internally at 192 Ashmore Road, Benowa QLD 4217. 

·  Rewards will be sent via a selected courier service to the referring patient’s provided address via normal postage.

·  BDT takes no responsibility for lost, stolen, or damaged gift cards. Replacement of lost or stolen gift cards will not be provided under any circumstances.

General Conditions: 

BDT reserves the right to disqualify any participant who does not comply with these terms and conditions or engages in fraudulent activities. 

Data Protection:

The decision of BDT regarding any aspect of the promotion is final and binding. Data Protection: 

Personal information collected in connection with this promotion will be handled in accordance with BDT’s privacy policy available on https://www.boweldoctors.com.au/privacy-policy/ and only used for referral tracking and gift card distribution purposes.
